Planting a Plant

As we age,  we can start to face physical, cognitive, and emotional challenges that affect our ability to perform daily tasks and maintain independence. We can work with you to help you continue to live a meaningful and fulfilling life by promoting safety, mobility, and participation in the things that are important to you.

We can assess your abilities and your environment to maximise your personal goals.  This could be around self care activities such as washing, dressing and managing a healthy diet.  Or it could be to support you with any leisure activities you may have, or be interested in.  Ageing also shouldn't prevent us being productive should we still want to be, and so we can support you to continue with activities at home, such as cooking as well as activities outside of the home such as work or volunteering.    We can recommend equipment and/or home modifications to reduce the risk of falls and make your home more accessible.  We can also review your moving and handling requirements so that you and your carers are safe.

We can also plays a core role in cognitive rehabilitation, helping you manage memory loss or other cognitive changes.  We can offer techniques for improving focus, organising tasks, and maintaining independence. By promoting social participation, encouraging physical activity, and addressing emotional well-being, we want to help you stay active, engaged, and connected to their community.  Our holistic approach supports aging allowing you to maintain your autonomy and dignity.
